The RANZCOG Exam which stands for Fellowship examination of the Royal Australian and New Zealand College of Obstetricians and Gynaecologists (FRANZCOG) is the key assessment required to qualify as a specialist obstetrician and gynaecologist in Australia and New Zealand. It is designed to rigorously evaluate whether candidates have achieved the level of knowledge, clinical judgement, and professional competence expected of an independent specialist.

The examination is made up of two main components. The written examination assesses a candidate’s depth and breadth of theoretical knowledge across the full spectrum of obstetrics and gynaecology. This includes maternal and fetal medicine, general and subspecialty gynaecology, reproductive health, and relevant aspects of ethics, governance, and evidence-based practice. Candidates are expected not only to recall factual information, but also to demonstrate critical thinking, problem-solving skills, and the ability to apply current guidelines and research to clinical scenarios.

The second component is the structured oral examination, commonly delivered in an OSCE (Objective Structured Clinical Examination) format. This part focuses on practical, real-world clinical competence. Through a series of stations or structured discussions, candidates are assessed on clinical reasoning, decision-making, communication skills, and professionalism. Scenarios may involve managing obstetric emergencies, counselling patients, interpreting investigations, or addressing complex gynaecological conditions, all while demonstrating safe, ethical, and patient-centred care.

Together, these components ensure that successful candidates have not only strong academic knowledge but also the practical skills, clinical judgement, and communication abilities required to practise independently and safely as specialist obstetricians and gynaecologists within the healthcare systems of Australia and New Zealand.

  • PTP/CWH: The Procedural Training Program (PTP) requires a 150-question multiple-choice exam, moving to remote proctoring in 2026.
  • Subspecialty: Specialized, written examinations for areas like Reproductive Endocrinology or Maternal-Fetal Medicine.

Eligibility

Eligibility to sit the Royal Australian and New Zealand College of Obstetricians and Gynaecologists (RANZCOG) Fellowship examination (FRANZCOG) is subject to a number of academic, professional, and training requirements that ensure candidates are appropriately prepared for specialist-level assessment.

Firstly, candidates must hold a primary medical qualification that is recognised in Australia or New Zealand. In addition, they must have general medical registration with the relevant regulatory authority—either the Australian Health Practitioner Regulation Agency (AHPRA) in Australia or the Medical Council of New Zealand (MCNZ) in New Zealand—confirming that they are legally permitted to practise medicine.

Applicants are also required to be appointed to an accredited obstetrics and gynaecology training position, ensuring they are working within an approved training program that meets RANZCOG standards. This requirement reflects the College’s emphasis on structured, supervised clinical training alongside examination preparation.

From a postgraduate training perspective, candidates must have completed at least Postgraduate Year 2 (PGY2) or higher at the time of application. Furthermore, from 2027 onwards, there is an additional requirement for candidates to have completed a minimum of 12 months of clinical experience specifically in obstetrics and gynaecology. This ensures that examinees have sufficient hands-on exposure to the specialty, including managing obstetric care, gynaecological conditions, and relevant clinical responsibilities, before attempting the Fellowship examination.

Collectively, these eligibility criteria are designed to ensure that candidates entering the FRANZCOG examination process have an appropriate foundation of medical knowledge, clinical experience, professional registration, and supervised specialty training, thereby supporting both patient safety and the high standards expected of future specialist obstetricians and gynaecologists.
