FRACS

Fellow of the Royal Australasian College of Surgeons

Overview

The Fellowship of the Royal Australasian College of Surgeons (FRACS) Fellowship Examination is an intensive, high-level assessment administered by the Royal Australasian College of Surgeons (RACS). It represents a critical milestone in surgical training and is designed to confirm that a candidate has achieved the standard required for safe, independent practice as a consultant surgeon in Australia and New Zealand.

This examination is deliberately demanding, reflecting the complexity and responsibility of specialist surgical practice. It assesses whether trainees can function at a consultant level, demonstrating not only a deep understanding of surgical science but also sound clinical judgement, decision-making, and technical expertise. The examination process is divided into two principal components: written examinations and clinical and viva (oral) assessments.

The written papers evaluate comprehensive theoretical knowledge across the candidate’s chosen surgical specialty, including anatomy, pathology, peri-operative care, and evidence-based management of surgical conditions. The clinical and viva components focus on the practical application of this knowledge, testing candidates through case-based discussions, patient assessments, and oral examinations that explore diagnostic reasoning, operative planning, complication management, and ethical considerations. Across these formats, the exam is structured into a total of seven assessment segments, ensuring broad and in-depth coverage of the specialty curriculum.

The FRACS Fellowship Examination is typically conducted during two examination periods each year, usually in April or May and again in August or September. Successful completion signifies that the candidate has met the rigorous standards set by RACS and is fully prepared to practise independently as a specialist surgeon, delivering high-quality, safe surgical care within the Australasian healthcare system.

The FRACS Exam:

Exam Structure & Components

  • Written Examination: Conducted about a month before the clinicals, typically consisting of two papers. These include Multiple Choice Questions (MCQs) and short answer/essay questions, covering clinical scenarios and, in some specialties, image-based questions (e.g., x-rays).
  • Clinical/Viva Examination: A comprehensive, multi-part examination held over two consecutive days, often focusing on clinical decision-making, anatomy, and operative techniques.
  • Format: Candidates typically have 2 hours for each written paper. The oral components are usually conducted by pairs of examiners.

Key Details for Candidates

  • Location: Held in major Australian cities (Adelaide, Brisbane, Hobart, Melbourne, Perth, Sydney) and New Zealand (Auckland, Wellington).
  • Timing: The April/May 2026 exam registration opens in early January.
  • Standard: The expected level of performance is that of a consultant surgeon in their first year of independent practice.
  • Marking: A “closed” marking system is often used, where examiners reach a consensus score (1-4) for each segment, with 3 representing the required standard.

Preparation

  • Focus: Success requires deep technical knowledge, the ability to manage complex cases, and demonstrated professional judgment. Preparation often involves extensive practice with mock vivas and written scenarios.

FRACS Exam Eligibility

Eligibility to sit the Fellowship of the Royal Australasian College of Surgeons (FRACS) examination in Australia is restricted to doctors who are formally recognized within approved RACS training or assessment pathways. Candidates must either be enrolled as an accredited Surgical Education and Training (SET) trainee or be recognized by the College as an approved Specialist International Medical Graduate (SIMG).

Applicants are required to hold current general medical registration, allowing them to practice medicine legally in Australia (or New Zealand). In addition, eligibility is generally limited to Australian or New Zealand citizens or permanent residents, reflecting workforce and regulatory requirements within the Australasian healthcare system. Candidates must also have met all prerequisite training obligations set by RACS, which include successful completion of specified compulsory education modules, professional development activities, and any specialty-specific requirements.

Together, these criteria ensure that only doctors who have progressed through appropriate training pathways, demonstrated commitment to professional standards, and met regulatory and educational benchmarks are permitted to attempt the FRACS Fellowship examination. This maintains the high standard of assessment and ensures candidates are adequately prepared for evaluation at consultant surgeon level.

  • Clinical Examination (CE): A practical, 16-station Objective Structured Clinical Examination (OSCE) focusing on clinical skills.

International Context
While FRACS is specific to Australia/New Zealand, it is internationally recognized. Similar, but distinct, qualifications include the FRCS (Fellow of the Royal College of Surgeons) in the UK/Ireland.
