FANZCA

Fellowship of the Australian and New Zealand College of Anaesthetists

Overview

The Australian and New Zealand College of Anaesthetists (ANZCA) specialist training pathway includes a structured examination process that trainees must successfully complete in order to qualify as specialist anaesthetists. Central to this process are two major, high-stakes assessments: the Primary Examination and the Final Examination. Together, these exams are designed to confirm that trainees possess the scientific foundations, clinical expertise, and professional judgment required for independent specialist practice.

The Primary Examination is usually undertaken in the earlier phase of training and focuses on the core scientific principles that underpin anaesthetic practice. It assesses a trainee’s understanding of subjects such as anatomy, physiology, pharmacology, pathology, and physics as they relate to anaesthesia, pain medicine, and perioperative care. This examination typically includes written components, such as multiple-choice questions (MCQs) and short-answer questions, which evaluate factual knowledge, conceptual understanding, and the ability to apply scientific principles to clinical scenarios. In addition, the Primary Exam often includes viva (oral) examinations, where candidates are required to explain concepts, interpret data, and demonstrate logical reasoning under direct questioning by examiners.

The Final Examination is undertaken later in training, once trainees have gained substantial clinical experience. Its primary purpose is to assess readiness for independent practice as a consultant anaesthetist. This exam places greater emphasis on clinical decision-making, procedural competence, patient safety, and professional judgment across a wide range of perioperative and critical care scenarios. Like the Primary Examination, the Final Exam includes written assessments—commonly MCQs and short-answer questions—to test applied knowledge. The viva components play a significant role, allowing examiners to explore how candidates approach complex clinical situations, manage complications, communicate effectively, and integrate scientific knowledge with real-world practice.

Overall, the ANZCA Primary and Final Examinations provide a comprehensive and progressive assessment of a trainee’s development. By combining written and oral formats, the examination system evaluates not only what trainees know, but also how they think, reason, and perform in clinically demanding situations. Successful completion of both exams is a key requirement for achieving Fellowship of ANZCA and recognition as a specialist anaesthetist in Australia and New Zealand.

The ANZCA Exam:

Exam Structure & Components

  • Primary Exam: Taken earlier in training, this focuses on basic sciences (physiology, pharmacology, anatomy) and foundational knowledge. It includes an MCQ component.
  • Final Exam: Taken after at least three years of training. It is comprehensive, covering clinical anaesthesia, pain management, and intensive care.
    • Written: Consists of 150 MCQs (150 minutes) and 15 Short Answer Questions (SAQs, 150 minutes).
    • Vivas: 2 medical vivas (15 mins each) and 8 anaesthetic viva scenarios (15 mins each).

Key Details for Candidates

  • Format: The exams are rigorous and test decision-making in complex clinical scenarios.
  • Eligibility: Must be an ANZCA trainee in good standing. Specialist International Medical Graduates (SIMGs) may only need to sit the viva components.
  • Regulations: No electronic devices, timekeeping devices, or personal calculators are permitted inside the examination room.

ANZCA Exam Eligibility

Eligibility to sit the ANZCA examinations is closely regulated to ensure that candidates have sufficient medical experience, structured training, and ongoing engagement in the anaesthesia training program before undertaking these high-stakes assessments.

To be eligible, candidates must be registered medical practitioners, holding current and appropriate medical registration that allows them to practise clinically in Australia or New Zealand. In addition to registration, trainees must have completed a minimum of 104 weeks of approved training, which includes both pre-vocational experience (such as internship and early resident years) and vocational anaesthesia training undertaken within accredited posts.

Within this overall training requirement, at least 88 weeks must be full-time equivalent (FTE) clinical anaesthesia training. This ensures that candidates have substantial hands-on exposure to anaesthetic practice, including perioperative patient care, procedural skills, and management of anaesthesia-related complications. Time spent in non-anaesthesia roles may contribute to the total training requirement but cannot replace the required amount of direct anaesthetic experience.

Applicants must also have successfully completed the introductory or basic training requirements set by ANZCA. These early training stages are designed to provide foundational knowledge, core clinical skills, and familiarity with safe anaesthetic practice, all of which are essential before progressing to the examination phase. At the time of application, candidates must be actively enrolled in the ANZCA training program, demonstrating ongoing participation and satisfactory progress.

Finally, all administrative requirements must be met. This includes payment of all relevant training and examination fees and compliance with college policies and deadlines. Together, these eligibility criteria ensure that only appropriately trained and currently engaged trainees are permitted to sit the ANZCA examinations, supporting the integrity of the assessment process and maintaining high standards for specialist anaesthesia practice.
