FAFOEM
Fellowship of the Australasian Faculty of Occupational and Environmental Medicine.
Overview
The AFOEM Exam Stage A and Stage B is conducted under the auspices of the Royal Australasian College of Physicians (RACP), form the core assessment framework for training in Occupational and Environmental Medicine (OEM). Together, these examinations are designed to progressively evaluate a trainee’s development from foundational knowledge to advanced, practice-ready competence in this specialty.
Stage A is typically undertaken earlier in training and focuses on assessing a candidate’s core theoretical and scientific foundations in occupational and environmental medicine. This stage is delivered as a written multiple-choice question (MCQ) examination, which allows for broad and objective assessment across a wide range of topics. The exam covers fundamental principles of OEM, including occupational health hazards, toxicology, epidemiology, workplace risk assessment, ergonomics, environmental exposures, and relevant clinical conditions. It also tests the scientific basis of practice, such as research methods, statistics, and evidence-based medicine, ensuring that trainees have the essential knowledge required to progress to more advanced clinical responsibilities.
Stage B is undertaken later in training and assesses the trainee’s ability to apply knowledge in complex, real-world settings. This stage includes a written examination that focuses on advanced clinical management, medico-legal and regulatory considerations, workplace systems, and organisational health issues. Candidates are presented with realistic occupational and environmental health scenarios and are required to demonstrate sound clinical judgment, problem-solving skills, and an understanding of legal, ethical, and workplace obligations.
In addition to the written component, Stage B includes a practical Objective Structured Clinical Examination (OSCE). The OSCE is designed to assess hands-on clinical skills, particularly history-taking, communication, and professional interaction in occupational health contexts. Through structured stations, candidates demonstrate their ability to gather relevant information, assess work-related factors, communicate clearly with workers and employers, and manage sensitive or complex situations.
Overall, the Stage A and Stage B examinations provide a structured and progressive assessment of both knowledge and practical competence. Successful completion of both stages confirms that trainees have achieved the clinical, scientific, and professional standards required for independent practice in occupational and environmental medicine.
AFOEM Exam Eligibility
Eligibility to sit the AFOEM examinations is linked to formal enrolment and progression within the Royal Australasian College of Physicians (RACP) Advanced Training Program in Occupational and Environmental Medicine. These requirements are designed to ensure that candidates have appropriate training exposure, academic preparation, and professional supervision before attempting each stage of assessment.
To be eligible for the Stage A examination, trainees must be officially registered in the RACP Advanced Training Program and have completed a minimum of six months of certified advanced training in occupational and environmental medicine. This training must be undertaken in an accredited training position, with a workload of more than 20 hours per week, ensuring sufficient clinical and workplace-based experience. Trainees must also have an approved supervisor, who provides oversight, guidance, and formal assessment of progress, confirming that the trainee is developing the foundational competencies required for Stage A.
Eligibility for the Stage B examination has additional and more advanced requirements. Candidates must have successfully completed the Stage A examination, demonstrating adequate foundational knowledge. They are also required to have completed an Advanced Life Support (ALS) course, reflecting the need for competence in managing medical emergencies in occupational and remote settings. In addition, candidates must hold a graduate diploma or higher qualification in a relevant discipline—such as occupational or environmental health, public health, or a related field—providing evidence of advanced academic training that supports higher-level clinical reasoning and professional practice.
Together, these eligibility criteria ensure that trainees attempt each examination at the appropriate stage of their professional development, with adequate clinical experience, academic preparation, and supervisory support. This structured approach helps maintain high standards in occupational and environmental medicine training and assessment.
